Please read this document carefully before registering or using the form6 platform. By using this platform, you confirm that you have read, understood, and agreed to all terms stated herein.

Acceptance of Terms of Use
By accessing, registering for, or using any services provided by the form6 platform, you automatically agree to be bound by the terms and conditions set out in this document. These terms constitute a legally binding agreement between you as the user and the form6 platform.
If you do not agree with any part of these terms, you are advised not to use the platform. Continued use following any amendments to the terms will be considered acceptance of the updated terms.
This platform is intended exclusively for users who are 18 years of age and above and who are located in a jurisdiction that permits the use of online entertainment services. Users are solely responsible for ensuring their participation is lawful under local regulations.

Account Creation & Security
To access the full range of services on the form6 platform, users are required to register an account using a valid Malaysian mobile number and a self-chosen password. Each user is only permitted to hold one account only. Holding more than one account is prohibited and may result in all associated accounts being suspended or closed.
Users are fully responsible for maintaining the confidentiality of their account login details. You are advised to:
- Never share your password with anyone.
- Use a strong, unique password that is not shared with any other account.
- Log out of your account when you are done using the platform, especially on shared devices.
- Notify our support team immediately if you suspect any unauthorised access to your account.
Form6 reserves the right to verify a user's identity at any stage through additional verification steps. Failure to complete verification within the specified timeframe may result in a temporary account suspension.
User Responsibilities & Rules
Form6 platform users agree to use this service solely for lawful personal entertainment purposes. The following actions are strictly prohibited and may result in immediate account termination:
- Using automated software, bots, scripts, or any other method to access or interact with the platform without authorisation.
- Attempting to hack, disrupt, or compromise the security of the platform's systems.
- Providing false information during registration or the verification process.
- Using another person's account or granting others access to your account.
- Taking any action intended to manipulate game results or betting outcomes.
- Using the platform for any activity that violates Malaysian law.
Users are also responsible for ensuring they are not located in a jurisdiction that prohibits participation in online entertainment activities. form6 is not liable for any legal implications arising from the use of the platform in restricted regions.
MYR Payments, Deposits & Transactions
All financial transactions on the form6 platform — including deposits, bets, and withdrawals — are conducted in Malaysian Ringgit (RM / MYR). The platform does not support foreign currencies, and no currency conversion is required.
Available deposit methods include TNG eWallet, Maybank2u, CIMB Clicks, and DuitNow QR. Each method has its own minimum limits and processing times, which may vary. Users are advised to check the latest information on the deposit page before making a transaction.
Regarding withdrawals:
- Withdrawals may only be made to a bank account or e-wallet registered in the same user's name.
- Withdrawal requests are processed within the timeframe stated on the platform — typically within 1–3 business days, depending on the method selected.
- The platform reserves the right to request additional verification documents before approving withdrawal requests that exceed a specified amount.
- Any attempt to commit fraud in transactions will result in immediate account freezing and further action being taken.
All transactions are final and cannot be reversed once confirmed, except in cases of clear technical error requiring a reversal. Any transaction dispute must be submitted to customer support within 72 hours of the transaction date.
Liability Limitations, Intellectual Property & Third-Party Links
Liability Limits
The form6 platform is not liable for any direct, indirect, incidental, or consequential losses arising from the use of or inability to use the platform, including service disruptions, data loss, or financial losses resulting from game or betting outcomes.
The platform strives to maintain continuous service availability, but cannot guarantee that it will be free from interruptions, technical errors, or scheduled maintenance. Users will be notified of planned maintenance whenever possible.
Account Policy — Suspension and Closure
Form6 reserves the right to suspend or close a user's account without prior notice if the user is found to have violated these terms, engaged in fraudulent activity, or upon request by the relevant authorities. Any legitimate funds remaining in a closed account will be processed for return in accordance with the platform's internal procedures.
Intellectual Property
All content on the form6 platform — including logos, designs, text, graphics, software, and interfaces — is the exclusive property of the platform. Users may not reproduce, distribute, modify, or use any such content without prior written permission.
Third-Party Links
The platform may contain references to third-party services or information. form6 is not responsible for the content, privacy policies, or practices of any third-party websites. Users access any external sources at their own risk.
Changes to Terms & Applicable Law
Form6 reserves the right to update, amend, or replace any part of these terms at any time. Users will be notified via an on-platform notice or registered email when material changes occur.
It is the user's responsibility to review this page periodically to stay informed of the latest version of the terms. The effective date of the current terms will always be stated at the top of this document.
These Terms and Conditions are governed by and construed in accordance with the laws of Malaysia. Any disputes arising from or in connection with the use of this platform shall be resolved in a court of competent jurisdiction in Malaysia.
If any provision of these terms is found to be invalid or unenforceable by a court of competent jurisdiction, that provision shall be deemed severed, and the remaining provisions shall continue in full force and effect.
